CARE COORDINATOR - REMOTE

CARE COORDINATOR Position Summary: The Care Coordinator is responsible for meeting with clients in their homes to review the DHS person-centered service plan (plan of care) and perform an initial assessment with the clients, develop the aide service plan; and revise client service plans following their annual functional (re)assessment or change in condition.
Essential Job Duties: Meet with clients in their homes to discuss person-centered service plan (plan of care).
Perform initial assessment with clients.
Explain processes for Palco Choice, Inc.
Primary point of contact for client.
Works closely with the RN and PCA Supervisor on plan of care for clients.
Revises client service plan, in conjunction with the RN, following the client’s annual functional (re)assessment or change in condition.
Frequent travel within a designated region of Arkansas.
Skills and Qualifications: Good communication skills, both verbal and written communication.
Good listening skills.
Must display a patient, caring disposition.
Ability to work with diverse groups of people.
Possess a professional demeanor.
Proficient in Microsoft Word, Excel, and Outlook.
Must have reliable transportation.
Must have a valid, unexpired driver’s license.
Must be able to pass a drug test, criminal background check, Adult Maltreatment background check, Child Maltreatment background check and MVR check.
Meet all requirements of the company’s remote work policy including maintaining a high-speed internet minimum of 50Mbps.
Frequent travel within the Benton, Washington and surrounding counties.
Education and Experience Associate’s Degree in a related field or High School Diploma and two (2) years of experience as a Personal Care Aide preferred.
Case Management experience preferred.
